On May 9, 2016, United Flight 1451 from San Diego to Houston suffered a bird strike at around 13,000 ft during its climb out:

After returning to Lindberg (KSAN), the aircraft was inspected, bird debris removed, and was cleared to make a second attempt at the flight to Houston.  However, it later had a navigation system failure and had to make an unscheduled landing at PHX for repairs.

Attached is the ATC communications before and after the bird strike.
