"In all cases the low pressure turbine stage 3 rotor had separated from the engine as result of material loss on the high pressure turbine blades, that caused vibrations of the high pressure rotor."

"The NTSB also recommends that General Electric should be required to redesign the low pressure turbine stage 3 disk to be able to withstand the vibration forces created by an unbalanced high pressure turbine rotor."
